Meredith, Izzie, Christina und Alex sind nun die Ausbilder der neuen Interns. George wiederholt das Jahr als Intern, was ihm am Anfang nicht gerade leicht fällt. Unter den neuen Assistenzärzten befindet sich auch die Frau, die Derek in der Bar getroffen hatte - Lexie Grey.
Alle wollen vor ihren neuen Schützlingen gut dastehen, doch gelingt das nicht allen. Es gibt einen Toten der Aufersteht, Bambi, eine Schwangere, die nach einem Unfall mit einem fehlenden Arm eingeliefert wird und einen älteren Herren, der immer Hunger hat.
Am Ende werden Meredith und Derek eine "Entscheidung" treffen.
